Mission Statement
The Community Health Clinic’s mission is to provide basic health care, dental care , prescription assistance, advocacy, and education to the uninsured and underinsured residents of Hardin, LaRue, Meade, Breckinridge, & Grayson Counties in order to enhance access to health care in these five counties of the Commonwealth of Kentucky.

Eligibility
- **NOW SEEING CHILDREN**
- Must be a resident of Hardin, LaRue, Meade, Breckinridge, or Grayson
- Must be UNINSURED
- Receiving disability income WITHOUT medical coverage
- Must meet income eligibility guidelines
- At or below 300% of the Federal Poverty Level (FPL)

*Basic health care to those who qualify.
*Health counseling & advocacy
*Extensive care coordination
*Chronic disease management
*Mental Health Services
*Individual diabetes education
*Physical Therapy
Other Services
*Prescription medication assistance to the uninsured/underinsured
*QHP, Medicaid, SNAP, & CCAP enrollment & advocacy
(health insurance, food stamps, and child care assistance)
*Medicare enrollment and plan review & renewal
*Assistance with durable medical equipment
*On-site licensed volunteer pharmacy
